- If you clicked on Activate Code account, on the new page enter the following details: MyChart Activation Code, Date of Birth, ZIP Code and Primary phone number.
- After entering the required details, click on Next.
- Now, enter a username and password and then click on Create My Account.
- If you do not have an activation code, then select the SIGN UP NOW option to start activating your account online.
- Once you land on the new page, enter the information as asked on the screen including your First, middle, and last name, specify your country of residence, street address, date of birth, gender, email address, mobile phone number, and home number.
- After entering all the information, click on the box I AM NOT A ROBOT, then click on Next.
- The system will verify your information.
- If you are verified, then you will need to enter your username and password.
- Then click on Create My Account.
